Begin by determining the patient’s current metoprolol dose. We generally recommend a bisoprolol dose approximately one-third to one-half that of the metoprolol tartrate dose.
For example, a patient taking 100mg of metoprolol tartrate daily might transition to 25-50mg of bisoprolol daily. For patients on metoprolol succinate, the conversion is slightly different. Use approximately one-quarter to one-third the metoprolol succinate dose. So, a patient taking 200mg of metoprolol succinate might switch to 50-67mg of bisoprolol daily.
Important Note: These are guidelines; individual patient response varies. Titrate the bisoprolol dose gradually, closely monitoring blood pressure and heart rate. Always prioritize patient safety and tailor the dose to the individual’s needs. Consult relevant clinical guidelines and consider patient-specific factors like age, renal function, and other comorbidities when making dosage adjustments.
Adjustments may be necessary. Start with a lower dose and incrementally increase it until the desired therapeutic effect is achieved, while carefully observing for any adverse reactions. Regular monitoring by a healthcare professional is crucial during the conversion and subsequent treatment with bisoprolol.
